Thank you for choosing us for your Microblading or Plasma Laser experience. Please make sure you read the following instructions to assure the best results for your procedure. ​ It is very important to refrain from Alcohol Aspirin, or aspirin products, such as blood thinners. Please refrain from ​ Ibuprofen and Aleve for 24 hours prior to your appointment. The only product for aches and pain that will not make you bleed is Tylenol. Refrain from Vitamin E and fish oil capsules for 7 days prior to your appointment. ALL of these make you bleed excessively. Excessive bleeding during the procedure will negatively affect the longevity of your semi-permanent makeup application. In some cases, the application will need to be prematurely stopped. Please avoid Energy drinks & coffee for up to 24 hours prior to your appointment. Not having caffeine in your system will help you to relax much more easily, as well as help to relax the facial muscles in the areas we will be working on. Its also important to note all procedures are for 18 and over. Please consult your doctor about your health prior to booking. It is your responsibility to know the contraindications including allergies to numbing agents for all procedures. You will be required to comply with all above mentioned as well as consent forms prior to servies.

IMPORTANT CONTRAINDICATIONS MICROBLADING BROWS I am over 18 years of age and can provide a valid driver's license or ID card. I am not pregnant or breastfeeding. I do not have any history of herpes infection at the proposed procedure site. I do not have epilepsy, diabetes, allergic reaction to latex or antibiotics, hemophilia or other bleeding disorders. I do not have cardiac valve disease or suffer from any heart conditions or take medications that thins my blood. If I suffer from hepatitis, or other risk factors for bloodborne pathogens exposure, or any other communicable disease, I have informed the technician and have been advised of any medication and procedure to promote the satisfactory healing of my tattoo. I do not have a history of medication use or currently using medication, including being prescribed antibiotics prior to dental or surgical procedures. I am not allergic to anything that is to be used during my procedure. If I have had previous work done, I have already sent a photo for approval before booking. I do not have any keloids, and am not prone to keloid scarring I have not had any previous work done on my brows and if I do, I have sent a photo for approval I have read the Policy and agree to the terms and conditions. PLASMA LIFT SKIN TIGHTENING PLEASE READ BEFORE BOOKING YOUR PLASMALIFT / FIBROBLAST TREATMENT! Plasma-lift skin tightening is generally suitable for most people, as it is particularly effective at treating notoriously difficult-to-treat areas, including around the eyes or mouth, forehead, nose, neck, breasts, stomach, arms, knees and elbows, scars or sun spots, skin tags or anywhere on the body with extra/sagging skin. It is also suitable for most skin types and textures, although here are some contraindications: - If you have darker skin than a type 3+ on the Fitzpatrick scale. - If your skin is prone to scarring/keloids - If you have any of the following; severe (active) acne, lupus, psoriasis or vitiligo - If you are pregnant, breastfeeding, immune-deficient or have any other autoimmune disorder - You must ensure the skin is it's natural color and not altered by the sun during the time of your treatment. There is no pre-treatment required, but it is advised that you do not apply any cosmetic products on the treatment area, especially anything containing alcohol. No use of accutane within 1 year of treatment. Please avoid the following for 4 weeks prior to your plasmalift session: 1.Botox and/or fillers (in the forehead/eyebrow areas) 2.Chemical Peels and laser treatments 3.Tanning or heavy sun exposure

Aftercare instructions are provided to achieve optimal results, but there is no guarantee made as we have no control over what happens during the healing process. Please note children are not allowed to attend your appointment for safety reasons. No guests are allowed. Prior permanent makeup must be approved. If you have ever had permanent makeup before you must let us know prior to booking. Previously faded work of microblading will be treated as a new service. A $100 NON-REFUNDABLE DEPOSIT IS REQUIRED TO BOOK All APPOINTMENT, PLASMA LIFT TREATMENT, OR MICROBLADING SERVICES. ALL SALES ARE FINAL. NO REFUNDS. All appointments require a deposit to secure a booking that goes towards your appointment. Deposits are non-refundable but can be transferable to reschedule an appointment if a notice of 48 hours prior to initial appointment is given. Only ONE time rescheduling is allowed. Failure to do so will result in forfeiture of your deposit. Please double check that all of your contact info is correct when booking, so you can receive the proper notifications. We are flexible with rescheduling as long as it’s done within 48 hours prior to the appointment ​ ALL INITIAL PERMANENT MAKEUP SERVICES INCLUDE A COMPLIMENTARY 6-8 WEEK TOUCH UP

​HERE’S HOW TO PREPARE YOUR SKIN PRIOR TO THE PROCEDURE Thank you again and we look forward to seeing you. You're one step away from having FLAWLESS BROWS brows!!! 1 Month Before - No facials for 2 weeks prior - No Botox or Fillers 4 weeks prior 1 Week Before - No Tanning or intense Sun Exposure - No Retinoid products - No Tinting, Tweezing, Waxing or Electrolysis 24 Hours Before - No alcohol - No caffeine (coffee etc.) - No Vitamin E or Fish Oil Supplements - No excessive sweating or working out To achieve the most beautifully natural brows for your face, you will get the absolute best results by prepping for the appointment and following the after care. The eyebrow area must be healthy, strong, non-sensitive, and non-irritated. Aftercare Proper aftercare is extremely essential and makes 50% of the total treatment success. It is crucial to follow the advice given and aftercare directions, failure to do so may put you at risk of losing up to 80% of the drawn strokes. At the end of your session, you will be provided with an after care kit that has everything you need for the 7-10 day healing process. Skin types may vary, but 10 days are recommended for optimal results. ​ ​ The Healing Process WHAT TO EXPECT IN THE FIRST 6-8 WEEKS: ​ The first week is going to be a difficult process. Here’s a list of thoughts that will probably cross your mind: ​ Uh-oh these are way too thick and way too dark What have I done??? Why are my brows staring back at me? I look crazy, I can't go out like this... The great news is, the slight redness you see and the very intensified color are all part of the initial stages of healing and will only be from days 2-5. Please don’t worry, don’t freak out.. just be patient. In this initial shock phase we ask that you trust the process. After 7 days, the brows will fade about 20% and slightly shrink about 10% which will form a much more natural look. The sensation can sometimes be comparable to that of a slight burn and minor flaking or peeling may take place. Please refrain from picking at your brows as you may run the risk of removing pigment during your healing process. Healing will differ with skin type, you may experience loss of hair strokes and fading of pigment color. This is completely normal and why we will include a complimentary touch up session. During this touch up session your brows will be evaluated as well as your healed results. We will fill in any missing hair strokes and freshen up the color to your liking. Lastly, the best follow up timeline will be recommended for future appointments. ​ ​ I'M READY TO BOOK
